MEMBERSHIP FORM

 

SC COAC is a non-profit 501-© (3) organization, which has been organized to bring together adoptable children with permanent families through education, support, and advocacy. 

 

We need your support.  SC COAC must have a strong membership base to effectively continue its work on behalf of children who wait.

 

All members receive The Bridge quarterly and a discount on conference registration fees. 

 

  I (We) would like an individual/family membership:

 

$35 – annual    $40 – Premier Membership (includes COAC T-shirt) Size                 

  $100 – three-year      

 

We want an organizational membership:

 

              $125 – annual            $300 – three year
